"Hi, I'm Dr. Bruce Dorn from Eye-Revive.ca. An extremely common question in my office is what is dry eye?
Dry eye, simply put, is lack of production of a normal tear film on the surface of your eye. The normal tear film is incredibly important for two reasons... One, having a normal smooth tear layer allows proper refraction or light to travel through the eye properly so you can have clear vision. Secondly, the cornea is very unique in that it's one of the only tissues in your body with no blood supply. So, you need that oil layer to literally feed oxygen to the cornea.
The tear layer is a lot more complicated than we might imagine. There are three distinct layers. The outer layer is an oily layer, the central layer is an aqueous layer, and the very bottom layer or the layer adjacent to your eye or your cornea is called the mucan layer. Any dysfunction or any one of those three layers can cause a chronic dry eye condition, and by its very nature because it occurs every day, day in and day out, becomes very, very problematic for any patient who experiences this."

What is Blepharitis
Blepharitis is a very common inflammation of the eyelid margin right where the eye lash grows out of your eyelid. There are two basic types of blepharitis. The first one, anterior blepharitis occurs on the very outer part in the lash, or the lid where the lashes grow out… it's generally caused by staphylococcus bacteria that infects the follicle. Second type of blepharitis is posterior blepharitis, and that occurs closer to the eye where the eyelid touches the margin of the eye, and it occurs in the oily meibomian glands. There's a dysfunctional of these meibomian glands caused by either acne rosacea or demodex which is a small mite that can infect the follicles or scaly flakey skin around the eyes that can fall in the eye and create a type of inflammation.

The cornea is the only part of the anatomy that has no blood supply—thank goodness, it would be difficult to see through red! The body has invented a way of feeding oxygen to the cornea because of this lack of blood supply. Small glands in the lids called Meibomian glands produce a complex mixture of oils that build up in a layer(or puddle) on the bottom lid margin. Approximately 22 times a minute, the upper lid comes down to meet the lower lid (blinking). The upper lid then spreads a smooth layer of oily tear over the front surface of the eye. This oily tear has some very critical functions:

In recent months, there has been an abundance of clinical trial research being conducted on dry eye treatment and blepharitis treatment. Today, we are treating more and more patients who suffer from itchy, red and irritated eyes. With so many treatments out there, it’s hard to know which treatments are effective and which therapy is right for you. Let’s take a look at recent studies and research done in the field of eye hydrating therapy.
In the July 2012 issue of Optometry & Vision Science, an interesting article examining the efficacy of traditional hot compresses versus moisture release therapy devices has garnered a great deal of attention. A study was conducted in which twenty subjects were assessed after applications of hot compresses and again after the subjects had worn eye hydration devices. The results indicated that eye-lid hydration devices proved to be more effective in providing warmth and hydration. Moreover, visual acuity and eyelid redness was improved after the subjects had worn the hydration devices, but not after the subjects had applied hot compresses. Rather than resort to hot compresses or at-home remedies, we suggest trying hydration goggles that allow you to sleep comfortably, and provide all-night hydration for your red, puffy eyes.
On an episode of The Doctors, a patient suffered from chronic dry eye symptoms that required her to take eye drops, apply compresses, change her sleeping positions and patterns and alter her lifestyle. It is not uncommon for many patients to try a variety of treatments to relieve their burning, sore eyes. The Doctors suggested, in addition to other treatments, that she wear eye hydration devices as a long-term solution. Contacts can often worsen the condition, but sometimes wearing contacts is unavoidable. Today, there are many lubricating dry eye drops, often referred to as rewetting drops, that provide soothing relief for contact lens wearers. However, sometimes these products are not as effective as more medicated, ophthalmic drops.
There are many misconceptions about wearing contacts and using eye drops - some say it’s fine to use regular eye drops while wearing contact lenses, but it can actually be damaging your eyes and lenses. Here are a few tips on how to use eye drops for dry eyes if you wear contact lenses.

If you are wearing contact lenses, remove them and place in a contact lens container with contact lens solution. Lubricating dry eye drops work best when not wearing your contact lenses. Contact lenses can act as a sponge, soaking up anything you put in your eye, including chemicals not meant for contact lenses. Damage to the tear film can occur, as well as redness or infection.
Wait approximately 10-15 minutes before putting your contacts back in, to avoid damaging the lenses or affecting the effects of the dry eye drops. Remember to always remove contacts before applying drops of any kind.
Using dry eye drops with contact lenses can cause irritation, infection, and in some cases, allergic reactions. These conditions can worsen and even cause permanent problems.

]]>Blepharitis is an ocular condition that affects the eyelid, particularly the area surrounding eyelash follicles. Blepharitis as actually relatively common, and can be contracted in a number of ways. The condition is typically defined by inflammation of the eyelids. Anterior blepharitis affects the outside of the eyelid, while posterior blepharitis concerns the inside of the eyelid, in contact with the eye.
Blepharitis is caused by an overgrowth of bacteria. Too much oil buildup, acne rosacea, dandruff, lice, allergies or seborrhoeic dermatitis may also contribute to the symptoms. Typically, when the oil glands near the base of the eyelashes produce too much, too little or the wrong balance of oils, the follicles and skin surrounding the follicles become irritated.
Discussion of symptoms, medical history and an occasionally bacteria swab is enough to diagnose a patient with Blepharitis. A quick eye examination is usually all it takes to determine the severity of the condition.
Blepharitis is treated through careful cleansing of the scalp, face and eyes on an ongoing, daily basis. Comforting and soaking the eyes with hot compresses several times a day will allow the follicles to open up and relieve the eyes of any blockages which may cause a crust to form and flaky skin to develop.
